our family experience

Apartment size is perfect, plenty of space to downsize but be comfortable. Master bed and Bathroom are very spacious! Patio is big enough for a couple of chairs to sit outside. Nursing staff has been hard to reach at times. [name removed], the move in coordinator has been a great help!! She was very communicative through the process and still is. The twice per week shopping trips are great, allows my parents some freedom to shop for themselves / enjoy some independence

Should have stayed the 3 months, but wanted to go home. I could do enough for myself to go home.

Lots of activities and a large dining room, restaurant style. The staff is friendly and courteous and will assist you when necessary. Security is tight and maintenance comes when notified. Residents can come and go within the facility as well as with family on visits.

Overall recommend

Quality of care - No clue who to chat with when have questions. Left hand says one thing and the other says something else. Frustrating. This is new to us and while [name removed] is very very challenging, the rudeness isn’t acceptable. I don’t defend her attitude yet there is validity I have to try to address. However it goes in one ear and out the other with the caregivers. In one ear and out the other. With all staff. Administrative - no idea as nobody, nobody follows up with our questions, rarely cleans the apt, had to get ugly as the linens weren’t cleaned and the bed made up for three weeks. Food is very good, the menus and there is great variety. Activities - Very good but not accessible to those residents who need to be transported from apt to the dining room, the game room etc. Therefore [name removed] is left out of that loop -she has a wheelchair but must wait for a caregiver to come get her and thus she misses out. With her attitude, they deliberately make her last. And unfortunately she smarts off to them which doesn’t help and isn’t acceptable and I’m trying to work on it but, she does have some valid reasons and I unfortunately have to delicately intervene. Frustrating. 8. Value for the money -I personally think it’s worth it. [name removed] has never- ever- done anything on her own. Her husband waited on her 24/7 therefore she’s used to being catered to. He worshipped the ground she walked and she expects them to treat her like he did. She intentionally aggravates them if they aren’t on time.

Experience with The Fountains at Mill Cove

They were in the middle of remodeling and redecorating but still let me look all around. Very quiet on campus. Many trees so nice and shady. The admissions lady knew they were number 2 on my list but she still shared very helpful information and useful information on Medicaid waivers. I would recommend them to others who are searching for senior living for their loved one.

Pleased overall

The apartment is a large for an Assisted Living facility. Bedroom is roomy and bathroom is spacious and toilet and shower are separate from the sink. Staff is very caring. She feels completely comfortable with them. Her apartment is cleaned weekly and they change her bed sheets and do her laundry There are a lot of different activities available for her to choose from. She loves the food and the way other clients always invite her to join them. I have no complaints, my mother has dementia and still wants to return home. We are hoping she settles in soon. She wasn't taking care of herself alone.

My experience is good

All in all, it has been a fair experience. The facility is older but maintained fairly well. My only real complaint is the elevator being small and in need of renovation. Nursing attention, especially on weekends, is somewhat lacking. The room call buttons did not work in the apartment. We ended up getting a call button after complaining several times. Caregiver staffing is apparently lacking. Assistance calls are occasionally more than 40–90 minutes for bathroom assistance, with no response during staff breaks. Laundry is usually left in the dryer, then put in a basket and dropped off at the room. Everything is wrinkled. There is no internet or basic TV in the apartment. Activities seem to be sufficient and well attended. However, residents in wheelchairs who are unable to get themselves to the main floor and back to their rooms may have difficulty participating. Caregivers say they're not permitted to accompany residents, so those residents do not get a chance to participate in the activities. The apartment size is very adequate, and the food is good. Outside medical services are very responsive, and the personnel are courteous and respectful. These are the things that stood out in my observations. I am not well versed in the goings-on of assisted living facilities, but I think common sense would prevail that providing assistance in a timely manner would be a primary consideration for residents in the facility.

The Fountains at Mill Cove

The food is not well prepared, healthy, and easy to eat for older people, they are not good with medication, they let medication run out and then when it is ordered you cannot get your meds properly, there is little help from the CNAs, especially on the weekends, the personnel does not appear to be properly trained or followed up how they perform, most don't know how to do laundry and dry and fold clothes, clothes are given to wrong person and Socks are lost. The turnover of personnel is frequent for some reason. The response is very slow in case of emergency, as much as 2 hours or more at times. They do have good activity programs and entertainment programs. They keep it clean and trash removed frequently. The price is better than many others. More personnel training could make a big difference. I have addressed most of these problems with the assisted living people but no significant change has been made.

TIRED

They gave a great first impression. Mom finally agreed, at 97, to go into assisted living. Mom has been there 2 months & several things need improvement. 1. Not enough assistance getting to & from dinning rooms....Mom's unit couldn't be any further from them & still be in the building. 2. Not enough assistance getting/urging to bathe 2 times per week. 3. Laundered clothes returned damp....not folded nor hung up. 4. Just discovered that her bedding had not been changed since she moved in! 5. Her AC was out for 2 weeks! Reported it several times & had to threaten to withhold $ to get it fixed. 6. It took over a month to get her landline phone operational. 7. No help at all on getting cable TV set up...AT&T was no better! Bought a new TV/sound bar in order to get Geek Squad to install!! Am I happy?! What do you think?!

Mom is thriving!

We are doing great with The Fountains at Mill Cove. Thank you so much for asking. My mom is truly thriving there and really enjoying herself. The staff is incredibly friendly and helpful, she loves the meals and sense of community, and it’s been wonderful seeing her so engaged and happy. The rooms are a little smaller, but they are perfect for her needs. Overall, we’ve been very happy with the experience.

Horrible care- do not recommend

1 star (a zero if I could). It was horrible. All assisted living means to them is that they bring her pills. They would not help residents get to activities or engage them at all. The food was not good and usually cold. Room size was good but it was all very old and not kept up well. When they told her it was mealtime, mom would many times say no. So they just left her with no food. They wouldn't encourage her to eat and just claimed she has free will. She would have died within months of living there had we not moved her out.

They get 5 stars. Great staff. They always make sure (or at least attempt) to get my mom to eat. Dad loves the food. Plenty of activities. Facility is clean. Apartment size is quite spacious compared to some other facilities my brother and I initially toured. The DON is always nice to me, despite how difficult my parents are. Overall, I am extremely impressed with the care the Fountains is providing my parents.

My worst nightmare

My mother reached this facility today for Physical therapy [date removed] from a hospital in Jacksonville with a fracture on her spine. Her last medications where at 3pm at Baptist and still Fountains does not have her medications. My mother is in a lot of pain. They’re telling me she’ll have her meds tomorrow. My mom is on a very strong med for pain and also sleep and nausea meds and how they expect her to sleep without no medication for tonight and god knows when tomorrow. This is my worst nightmare coming true. No one wants to see their parent in pain or discomfort.

My experience at The Fountains Assisted Living

It is very clean, studio size is more than adequate, food is pretty good and there are plenty of activities, I did not give it a 5 because I think they are understaffed, there needs to be more than 1 CNA per wing/floor, especially if your loved one needs more assistance with their daily living activities than others.

If only they had more staff...

Overall, the Fountains Memory section as good. Staff: very important but inadequate. All the patients required differing amounts of care and attention. About 45 patients and there was probably only 4 medical aids most of the time. Many times I was looking for someone to give pain meds and no caretakers around. Even one person said she was on her break and would help out later. When Mom caught the flu there, it was almost impossible to get her any meds or help. After a few days of severe congestion and runny nose, Mom passed away. Activities I rated as 5. The gal in charge of that was super uplifting. Food/dining was about a 4, almost a 5. So many patients needed assistance, it was hard for them to keep up with. Cleanliness was a 4. Often times the halls smelled like urine, but they worked hard to clean things up. (Another example of the patients needing more attention.) Apartment size was great, a 5. No problems here. However, Access into and out of the memory care section was terrible (a rating of 1). Because of the necessary security, staff has to go to the access door to unlock the door every time a visitor comes and goes. The doorbell/buzzer could be heard throughout the entire facility disturbing everyone. Overall, I think the Fountains Memory Care section could be very good if they hired more medical/caretaker staff.

Tips for evaluating your community options

Safety is a primary concern when seniors and their families look for Assisted Living. To ensure that a community meets your needs and expectations, we recommend that seniors and family members:

  • Know Florida’s regulations for memory care. Each state has its own laws and inspection processes to ensure seniors reside in a safe environment. Depending on your state, you may even be able to look up inspection reports for the communities you’re considering. Learn more about Florida’s regulations for memory care and download the Guide to Senior Living Licensure and Inspection Reports.

  • Tour the location. Touring allows you to speak to current residents and staff, and it helps you confirm the accuracy of the community description and photos. Use our touring checklist to help guide your visit.

  • Confirm the availability of required services. Before committing, make sure that the community offers all the services your loved one needs, and that those services can be provided within your budget.
