I toured this facility. It is way too expensive for what you are getting. It is all private pay. There are basically no private rooms. Rooms are either shared with a dividing semi-wall or you have to walk through one bedroom to get to another bedroom. The residents did not look like they had many choices in their daily activities. They were grouped together for activities but discouraged from hanging out in their rooms. It looks like it was easier for staff to keep up with them if they were all together... There were dogs freely roaming around everywhere. (My family is not really into animals - so I did not care for this.) The staff were also encouraged to bring their pets to work.

I do not recommend this facility, I researched 8 different facilities and was highly impressed with Story Oaks. After a week she was calling us afraid, and when we visited her room was a mess and sheets were missing. We made a list of things that we wanted to be fixed in the room and it was never done. After 30 days we decided to pull her out of the facility, and moved her into a new facility, and is doing amazing.

They neglect residents unless the residents can function on their own and just over medication many of them. They don’t do activities with residents unless they are fully functioning residents. Then they pick only their favorites to participate. They also have a very high turn over of staff not one aide has been there even a year and staff lacks training. They also only have one cma for two buildings.

We moved my dad there in January 2017 from another facility. Storey Oaks is a "God-send". They are by far the BEST staff of any other homes!! They take such good care of my dad. You can tell the staff have all been trained. They all "touch". Just a touch on the shoulder to show you care goes a long way. They are all so respectful (calling my dad [Removed]). They show each patient so much respect (as if they were still in the "working world") The food is great & they get snacks everyday. I have eaten with my dad & it was very good. They are very patient with the residents and I have never seen any disrespect or roughness. The chef comes out of the kitchen and visits with them and knows the residents he is cooking for. The activities director always has them doing crafts, games, reading the paper, lots of fun things. She encourages them to participate in activities. The receptionist is awesome; always greets us with a smile and happy decanter. The administrator is the best I've seen. He gets out of his office and knows his residents and families. The community relations director is the most caring person. She is interested in the needs of the families as well as the residents. The entire staff ( from laundry, kitchen, nursing, cleaning, activities, to executive are there to make a good life for the residents. And it shows!!)The entire staff is the best thing about Storey Oaks. They are as respectful to us (the family) as they are to my dad. They have asked us what dad's interest are and want to get to know him and our family. Storey Oaks is very clean and has a wonderful lemony smell. I have NEVER smelled urine!! We have never lost anything, as they do all their laundry separately. Laundry is done often. Dad has great personal care EVERYDAY!! He is shaved, teeth brushed, hair combed, dressed every morning. I can't say enough about the staff. They care!! And it shows!! Best home in Okc!!

The Prairie Winds is very understaffed. It is clean and beautiful, but on more than one occasion they lost his glasses and he was wearing someone else's clothes. My father went in able to walk with his walker. By the end of the week he could not walk, then confined to a wheelchair.On a couple of days they couldn't locate his medicines, and had mislabeled his glucometer as someone else's. I was there everyday, and saw that the kitchen staff took five days to finally cut up his food so he could feed himself. He gave up trying on the fourth day. On his last day there (10 days) they called to have us come take him to be seen by a doctor. They said he fell out of bed, but the emergency room doctor said his injuries could not be caused by a fall. Dad had a broken collar bone and severe bruising on the whole right side of his chest and shoulder. He also had a defensive wound on his left lower arm. I saw his room that morning and the aide Izzy (he was great with Dad) had put his mattress on the floor as usual, and put the floor cushion beside that. He had only three inches to roll out of bed. We had enrolled him in hospice two days before that. They came to the hospital to transport him home, and had the hospital bed set up at home by the time we got there. Dad was glad to be home, but was in a lot of pain. Dad couldn't speak to tell us what happened, so we'll never know.

When you are confronted with putting a loved one in a facility, make sure you have somebody with you that isn't emotionally involved. My sister and I were faced with having to take my mom from the hospital to a facility. On Wednesday, [date removed], we took her to IRIS not seeing any other option. She had an appointment the following day with her PMC doctor who told me I should take her home and put her under Hospice Care. Less than 72 hours after she was admitted to IRIS we moved her back to her house as her doctor recommended and contacted Hospice. When I asked about getting a partial refund I was pretty much told, "Sorry Charley, No Refunds". I think it's absurd that for a stay of less than 72 hours cost me $6,000. The only thing these witches at this coven were interested in was getting me to sign the contract. I might also add, that the morning I went to pick her up, she had been left unattended in her restroom. She is a very high risk for fall. I'd never recommend this facility for anything.

My father was prone to falls. We were required by Heritage Point administration to hire a sitter full time at $18/hour on top of the facility's monthly fee. We recently relocated him due to the fact that this became too expensive as well as the fact that the staff expected the sitter to do the job we were paying for Heritage Point to do. We often had to request for his room to be cleaned and his laundry went for long periods of time without being done. Very rarely were activities done with the residents and the food was often sub par.

Very disappointed in this facility. The activities are minimal and the residents do not get the special care the staff advertises. Often they are left on their own--especially in the evenings and weekends. The nursing staff are not well qualified--there were too many mistakes in the dispensation of medicines. Clothing and hearing aids lost on a regular basis. The layout is beautiful and it is very clean but that does not make up for the minimal care the residents get.

I recently attempted to move my loved one into this facility. During the initial tour of the facility several promises were made concerning special equipment, to be specific availability of this equipment. (One day) Upon move-in and paperwork completion I was told these promises would not happen by a very rude nurse that obviously hates her job. After meeting with upper management about promises I was told sometimes things change. I would hope someone in their business would know better than to make promises that may not happen. I also expressed the need for someone professional to speak with concerning my loved one. Within two days of being at this facility it was obvious management could not communicate with each other or family members in a professional manor. Not a place I want to do business with or to have a family member in their care. On the third day I moved my family member to another facility. The personnel working with patients on the floor seemed caring but the people managing this place need to go away. Very unprofessional...

Not enough staff on floors for resdients. Bathrooms and rooms unclean and activities are not educated or consistent. Loved one is higher functioning, but due to understaffing and falling, is now declining quickly.

My mother was only in here for one or two months and during that short period of time 2 or 3 times I had to ask them to change her because it had been so long since they had changed her she was soaking wet so bad that even the front of her pants were soaked and she sat in a wheel chair. Once sitting in the front area with her a man grabbed her glasses off her pulling them sideways and I had to grab his hand to get them back while the lady working there just sat there. Another night they were having a lot of problems with a man going into a woman's room and her screaming for him to get out and leave her alone as I watched the workers just take their time walking to her room. Then they got him out and he came to where everyone sits and grabbed a hold of a ladies wheel chair telling her it was his and kept jerking it around and wouldn't let go. It took 3 women about 5 minutes to get him to stop and let go. Then they sat him down with everyone out front and every single person working the front went off down the halls to not be seen anywhere leaving all the women alone with him. Then the worst thing happened, my sister went to see my mom and noticed her foot was swollen and took moms sock off. My mothers skin was turning black in spots and her foot discolored. She had black sores on her foot, toe, and leg and not one person had notified my sister or brother who were in charge of my mothers well fare. My sister immediately got my mother to the hospital to find out she had a blood clot and her skin was rotting. It looked absolutely horrible and not ONE person notified anybody. Sorry to say 3 days later my mother died.

We put our Dad in there [date removed]. He was in there exactly 7 weeks before the Lord took him home. He was a high risk for falling. They assured us they would watch him and do everything they could to keep him safe. Dad was a very active person and you couldn't keep him still. Everytime we went in there he was wondering around by himself with no help or assistance. They would call and tell us he fell. The last 3 weeks he was sent to the emergency room 6 times. The first time they called my brother and said, your Dad is at the emergency room because he was jumping between 3 wheel chairs and we couldn't get him down. So he had 2 sets of stitches on his forehead and 1 right below the eyebrow. The second time he went, he had stitches on top of his head. He fell again and it was to late to do anything about it and it healed on its own. The 3rd time going to the emergency room they claimed he fell into a door handle and he ended up with 6 staples. The 4th time he supposedly fell over a chair and was complaining about his neck and was not responding well so they sent him to the emergency room. They couldn't find any damage to his neck or head so they sent him back. A couple days later they sent him back to the emergency room because they discovered that in the last fall he had broke 2 bones in his left hand and had 4 cracked ribs. The last time he went, my brother and I decided to take him because when I got there his eyes were glazed over and without jarring him around, he was unresponsive and nobody noticed until I got them in there to check him out. He was in the hospital for 3 days and totally against my desire we had no choice but to take him back to die. I stayed with him for 2 nights. The first night the guy that was his nurse took excellent care of him. The second night they were training a new nurse and the female nurse in charge of Dad was training the new girl. Dad was getting rid of his liquids in his body and needed to have a new pad under him anywhere from every 5 to 15 minutes. They did great until 1:30 am. Then they decided that mopping floors and doing other things were more important than taking care of a dying man and trying to make him as comfortable as possible. I waited on them for about 10 minutes to take care of him but they were nowhere in sight. We were out of pads that hospice brought, so I left the facility and drove about 4 or 5 miles away to Walgreens and bought pads. When I returned, I went to his room and changed his pads myself until 10 am the next morning. The graveyard nurses didn't show back up in his room until 5 am that morning. So from 1:30am until 5 am I was on my own. By the time they did come in, I was livid and told them, "do not touch him." If they couldn't even come in to check on him once in a while in 4 and a half hours, I can say by then I had it under control and didn't need nor did I want their help. He died that night while my brother was there. The other things that upset me are as follows : For whatever reason, Dad would urinate in the floor and tracked it all over the facility. When I asked them why they were not mopping it up, they told me they only mopped twice a week but they would turn in an order to mop every day. That worked for maybe a week and back to the smelly urinated floors again until the day he died. By this time he had a roommate that was sleeping on a mattress on the floor because he was falling out of bed. Not to mention the cruel care they gave that man the night Dad died. When they would call my brother to tell him why they sent Dad to the emergency room, they would tell him one story of what happened and tell me something completely different. Not one time did we ever get the same story twice. Maybe that's why they call it Storey Oaks! There are so many details I haven't even put in this review because it's to much to type. These are the worst of what happened.

They are very focused on profit and sales at this facility. You become aware of that the moment you arrive. As someone else had mentioned they are very disorganized. My relative has complained of some of the aides being very rough in handling of them. The relative has asked me to move them to another facility and has mentioned that the staff is very controlling and rude, which I have witnessed as well. In my humble opinion, this facility should be avoided. Do not let the new building distract you from quality of care issues.
