At Brookdale South Park, the dementia-trained staff members understand there is not a one-size-fits-all approach to memory care. Here, each individual can expect a personalized experience. This means every resident receives a tailored care plan with consistent care assignments.
There are two Brookdale memory programs available here: Clare Bridge and Solace. Although it was created more than 30 years ago, the Clare Bridge program continually evolves with new research, and the goal is to provide residents with six planned activities per day. These activities encourage residents to use their current skills, while preserving their abilities and true self. The Solace program does not replace any elements of Clare Bridge. Instead, it adds additional programming, such as music and aromatherapy, while providing assistance with bathing, dressing, and more.
Rest assured, staff members are available 24 hours a day, seven days a week to be alerted to an emergency. Additionally, residents benefit from community safety features, including a 24-hour security system and keypad entries. Assistive physical features are also present on-site, such as large circular paths designed to lead residents to areas of activity, as well as way-finding cues, and spaces that are decorated to feel like home.

I brought a friend that was diagnosed with dementia 2 years ago. I liked the facilities. I felt like it would be a safe place. My friend doesn’t feel like he is ready. The doctor will make that decision

My mother lived here for 1.5 yrs and we moved her out due to issues with staff. The main problem is the executive director,[Name Removed]. His management style is weak and ineffective. The staff seem to have no respect for him and it shows in the quality of care. Most caregivers are awesome (Names removed) but the last straw for us was finding a caregiver watching SpongeBob SquarePants on her iPad during dinner. Instead of helping residents, she chose to watch her cartoon while they waited to be served. Most of the residents on her wing weren’t even seated in the dining room as she left them to either watch tv or sit alone in their rooms. When it was obvious [Name Removed] was doing nothing to resolve the issue, we gave up. Our mom now resides at The Meridian at Carolina Lakes and pays [Amount removed] less per month for beautiful facilities that far outshine the outdated and drab building Brookdale uses. 1 star. I do not recommend Brookdale.

I would rank Brookdale South Park a 4. It was the least institutional feeling facility of the six I toured. All of the staff knows all of the patients and seem friendly and engaged. There are daily activities to keep residents involved. The food seems to be good.

It is pleasant, clean and the staff were pretty attentive and the food was good. The layout of the facility gives the residents plenty of space. This opinion is based on a rather limited time frame as they could not handle my wife's "aggressiveness", although they assured me before admitting her that they were fully trained and experienced with all behaviors.

I am shocked to pay $[removed] a month and to have a room big enough for a twin bed and a chair, to find our loved one in dirty clothes, hair not brushed teeth not brushed, poo under our loved one's nails..... and items constantly missing. It's heart breaking. But I have been told that even in the higher priced care facilities, that it's not any better. So I continue to go over at different times of the day for at least 4 days a week. We have a friend come in to make sure our loved one is clean and engaged.

My mom has been living in Brookdale for 9 months and I cannot be happier. The facility is warm & welcoming as [name removed]has a fabulous sense of style. [name removed] & [name removed]are always around to lend a helping hand and were so very helpful when we were setting up my mom’s room. [name removed]was also patient and great at helping with the paperwork. [name removed], [name removed], [name removed]& [name removed]have been my mom’s best friends since she arrived and I know those 4 were integral into helping her settle in so quickly. [name removed]does a wonderful job as the director of Nursing and I know mom is in good hands with her care. Overall, I cannot say enough good things about Brookdale. You will not regret moving your loved one here.

The LPN in charge of nursing and the caregivers, [name removed] is excellent, as is the activity director. Most of the caregivers were very kind and caring, but they need more caregivers. They were often short staffed, especially on weekends. The clients rooms are small and could be kept cleaner, but their philosophy is that the client residents should be out of their rooms and busy with activities and walking around most of the day. It is a very open plan, and residents are free to roam any and all of the many hallways and common areas. They are also able to go outside with supervision. They have beautiful grounds which are fairly large, with elevated flower/vegetable gardens who enjoy that.

The facility itself is clean and pleasant. The caregivers seem concerned and attentive. My wife was there for two weeks when she had an "incident" with either another resident or staff member. She was sent to the hospital to "have her medications "adjusted " . After three weeks she was assessed able to return to the facility. 20 hours later there is another incident and she is back in the hospital, this time for 6 weeks, during which Brookdale informed me that she could not return to their facility. I was left to find another memory care facility for her.

Everyone is friendly and helpful. The CNA's do their best, and seem very caring and kind, but residents need to have a degree of independence, rather than need 1:1 care as per state requirements there is an 8:1 caregiver to resident ratio. Meals and activities are good. Rooms do not have showers, but they have a nice private shower room. The Health Wellness Director, and Sales Manager are outstanding. The front desk personnel are also extremely helpful.

Brookdale South Park I thought was very dark (light wise) inside and we had a lady from our church that had a bad experience there. Plus, it was really too far from my house. These were the main reasons we did not choose this facility.

Very poor communication and assistance from [name removed], the sales manager. Tells me that she will call me, but never does. I have to keep reaching out to her. I ask her about staffing at community and she says I will need to reach out to nursing team to get the numbers. As a sales manager, I would expect her to have that information available to provide to prospective residents. Very disappointed at my interaction with her. If she is the representative of the community, I would not feel comfortable sending a loved one into this community.

This is a really nice community. It is well kept, with friendly staff. The staff communication with the family could be better. It would be nice to have the staff contact us when things happen with my loved one or changes. I would recommend the family keep in contact with the staff about your loved one. Absolutely I would recommend the community to others for their loved ones.

The actvities did not work out for my loved one. My loved one would, have loved to engage in more outdoor activites. The food options were not good, and was not a balanced diet, and my loved one was not getting vegetables The staff did the best to their ability. There was a turnover in staff but they always tried to make accomadtions-. Overall, I never had a negative experience.

If we could give Brookdale South Park 10 stars we would, we have been extremely happy with our loved ones care, we have nothing but great things to say about them. They have been so accommodating to our mother, they have gone well out of their way to keep us updated on everything that is going on. If she is ever having a bad day they will call us so we can come up and be with her. When she moved in and had to go through her 7 day quarantine they allowed me to stay with her the entire time. With us still having to quarantine we were set up in a space that had a living room, a nice kitchen and even a little space outside. They have a ton of different daily activities going on as well. We would highly recommend this community
