Overall, I wasn't completely satisfied with the level of care my mom received. I feel this community may be a better fit for individuals who are able to be fairly independent, as my mom required more assistance than was consistently available. I found myself visiting daily and providing a significant amount of support myself. There were also times when she chose not to go to the dining room, and the meal alternatives provided were fairly basic. When she did dine there, however, the food was plentiful. My mom was also receiving hospice services, and I was grateful for the additional support they provided during her time there. As her needs increased toward the end, I noticed that she received more frequent attention and monitoring, which I appreciated. I only wish that level of attentiveness had been more consistent throughout her stay. Overall, I believe the experience may be different depending on the individual's level of independence and care needs.

Seemed very very busy and a little chaotic at times. They had immediate openings was good, have better qualified employees would be better.

Q1: overall experience Like every facility, it's hit or miss. It never seems to even out after a rocky start. There's always some issue for me to dig into, complain about, get fixed as best I can Rating:3 Q2: cleanliness If housekeeping is staffed, its decent. Rooms are cleaned. Common areas, especially memory care dining, is almost always messy and sticky. Rating: 3 Q3: friendliness Some staff are great, some are awful: rude and uncaring. The ED often lies Rating: 2 Q4: quality of care Caregiver training isn't a thing. No CNAs anywhere, scary. Not much common sense. Also depends on who is working, whether a shift is properly staffed, holidays and weekends are always terrible Rating: 2 Q5: admin staff Ugh, the executive director is terrible. Corporate will not return calls. New memory care director seems sincerely determined to improve things Rating: 2 Q6: food Pretty good! Rating: 4 Q7: Activities Good for AL, not so much for memory care Rating: 4 Q8: value I wish it were cheaper based on what we get some days Rating: 3

When we started there in October everything went pretty well. The head nurse was promoted to director and no replacement was added. The caregivers are always on their phones and have chips on their shoulders about actually having to work. Conversations are very open and very loud in the common areas. About how they hate the place. The boss is always yelling at them. One day I cried after watching our security camera and listening to our roommate sobbing. She does not have dementia and the two caregivers helping her get to bed were actually talking about how nasty cleaning up her 'shit' was. A few days later they were cleaning up lunch and took plates of food with food still on them to get cleaned up faster for their break. One girl and a new girl were again saying how nasty cleaning up after 'these people' was and because they were feeding them greens these effing people were gonna be shitten everywhere. I cried again and took my mom to a different part of the building. I was approached by the 2nd shift supervisor asking why I left the room crying. Then we had 2 more unattended calls in the common rooms one of which was her cause of death. It's not unusual to have 2 caregivers and a single kitchen staff for the entire facility. The place is a mess and corporate didn't show up to 2 meetings and didn't bother to call back. In a 10 day period there were 7 falls, 3 deaths and several move outs. They do not let anyone use the patio as they are only fire doors so no one can go out without a code or supervision and there are not enough staff.

I was unhappy when some of the staff showed up in my mom's room one night. They did not know I was in the room. They came in to get drinks out of her refrigerator. I also received a phone call one night from one of the workers because of them having to clean up after her dog. She was quite nasty about it. My mom at that point was unable to help. I get the problem. When she moved in they had already called hospice and she was only there a few weeks before she passed away.

They were under staffed. The staff they had were on a break or going on break. The tech' s were more motivated. The director is miss leading. Because [Name Removed] was mobile she wanted him to go to assisted living 1- she had me having a care system come twice a day to empty his bag 2- there was no effort to remove the Cather 3- he was in danger because he was not monitored as I was told 4-he pulled out his Cather early one morning. Walking down the hall with it in his hand. Does that sound like what he needed? They got there[Amount Removed] [Name Removed] was not professional she was misleading

Rooms are recently painted and with some registered. Bathrooms are old and in need of major renovation. Few activity for memory care patients. They just sit them in front of the TV. Some upgraded fixtures

The place lacks good leadership. No Healthcare director and no Memory care director. Not enough staff. They do the best they can, most of them. The website talks about their MEMORY GLOW therapy, however, staff doesn't know what it is.

Room was big enough and had plenty of closet space and even a built in dresser which was nice. The staff was ok, some were really great and some were just ok. Dining room was nice and they gave a ton of food. My mother didn't go to the dining room a lot, but when she did she ate really well.

I would give Terra Bella a 5 rating. Putting a parent into assisted living is not an easy task and Terra Bella helped in every way possible. My brother and his wife met her and could not believe that her first meal was a gigantic juicy hamburger! I also like that you have choices if you don't like what's being served so that's always a plus! It takes a couple of weeks for a resident to get used to their surroundings. My mom has the beginnings of dementia as well as other health issues. Terra Bella offers physical therapy, occupational therapy, activities for the residents, field trips to go shopping, etc. i'm so glad my mom is there cause now I feel that she is safe and with other people, instead of being home alone.

I think the place is understaffed but that's probably the same situation as most facilities like it. I'm hoping my loved one gets the best care possible while he is there.

The new executive director was the head nurse. She is very easy to approach. She takes concerns very seriously and acts quickly. The new head nurse actually seems to care for the residents. Most of the staff appear happy to be there. The facility is more of a medical environment not so much a home feeling. Having a doctor, physical and occupational therapy in-house is awesome. The level of activities, especially for memory care leaves a lot to be desired. If there is an active loved one that needs socializing this would probably not be a good fit. They do have once a week outings and a weekly sermon other than that it's basically bingo and news. There aren't very many residents families that visit but those of us that do have a good connection with the staff and each other.

I had a good experience with Terra Bella Summit, although it got off to a rocky start. The person we were supposed to meet with was at the other facility, and so we got the official tour from someone else. We drove to the other facility and met with our original contact and chose to sign there for Terra Bella Summit. Overall I would give that experience a 4. One big thing I didn't focus on and wish I had: there isn't a portal or easy way to monitor things for my mom (like MyChart). I'm not sure any facility has this, but instead, I have to reach out to the director of health and wellness or call the front desk to talk to someone on staff. The director is fantastic, and I've only had good interactions with the staff. But I live 4 hours away, and if I could monitor things in a digital way, that would be great. I am happy to work with them the way we are working. I just did not know to ask about that type of thing, making assumptions of something like MyChart. You hardly go anywhere without something like that now.

I liked a few of the employees that seemed to care for the residents well. However, the management made a lot of promises and did not deliver. There were not any activities for the memory care residents. They need to have better supervision or night shifts. There are a few good employees, but for the most part a lot of laziness

The staff has been great! They answer all our questions and seem to genuinely care about the people in their care. It is extremely clean and very welcoming. I have to add that the folks we worked with through your organization were EXTRAORDINARY!! Mom's placement was unexpected and had become a crisis situation for us. We would not have made it without the help of A Place for Mom.
